What Are Forms?
Forms are structured surveys and assessments (e.g., RAI, MNA, the unit’s own form) that are saved in the client's information. Forms can be accessed from the client’s profile.
What Does Filling Out Forms Look Like?
Long forms (e.g., RAI) are divided into sections, and AI displays the different sections. The sections contain identifiers, followed by the AI-provided answer for that identifier. The answer can be numerical or text depending on the form. After the answer, there is a comment that serves as an explanation of the answer. The last column shows the form’s previous value, which illustrates whether there has been a change in the situation. If the previous assessment differs from the current answer, the section’s identifier is highlighted in yellow.
Filling Out the Form
- Open the client’s profile and go to the new form view.
- Select the desired form template by clicking fill out a new form.
- The AI will fill in the form with the client’s information where available.
- Review the pre-filled form one section at a time. After you have checked a section, you can transfer the selected answers from that section from Gilli to an external form system by clicking the transfer this section button.
Browsing Form History
Forms filled out for the client are saved in the client’s information. You can browse the history and open an individual form for review.
- Open the client’s profile and the new form view.
- Browse the list of forms—newest ones are typically at the top.
- Click a form to view it.
Tip. The form history is a good source of information when assessing the client’s situation over a longer period.
